New Registration

In order for your child to be considered officially registered at RCDN the below paperwork and documents need to be provided to the school. All items must be completed. Any items noted with a * indicates that the school must receive a hard copy of this document.

• Application

• Copy of Birth Certificate*: Required for all new students only.

• Parental Contract*: please keep a copy of the parental contract for your records and provide the school with a signed copy.

• Universal Health Record*: (must have doctor’s signature and date)

• Immunization Records*

• Medical Permission Slip*

• Allergy Forms*: These forms only need to be completed if applicable to your child.

• Permission Slip for Walking Trips*

• Parent Helper Medical Form*

• Committee and/or board position selections: Descriptions and Form

• $125 Non-refundable application fee

• One month’s advance tuition which will be applied to June’s tuition (3s - $350/$400, 4s - $450/$550)

• All adults planning on being a classroom helper, must be background checked and fingerprinted. The process must be started by the end of June (if completed for RCDN within the last 2 years you do not need to repeat)

For your convenience, please use this checklist to make sure you have all documentation and paperwork in order prior to turning it in to the school. Registration paperwork may be returned to the school at 32 Ridge Road. PLEASE DO NOT LEAVE CASH IN THE MAILBOX.
